Initial Checklist

CHECKLIST OF ITEMS TO BRING WITH YOU TO YOUR CONSULTATION - PLEASE BRING COPIES OF THESE DOCUMENTS, IF APPLICABLE

REAL ESTATE: All of the following: Deed, Title Insurance Policy, Closing Statement/Settlement Sheet, Homeowner's Insurance Policy, Tax Assessment, Loan Payoff(s), Proof of Current Value (appraisal or broker's price opinion), Foreclosure and Sheriff Sale notices

INCOME: All paystubs, unemployment compensation, disability income statement, social security, alimony/child support, and income from any other sources for the last six (6) months. If you have your own business, profit and loss statement and balance sheet for last six (6) months.

TAXES: Last four (4) years filed Federal and State Tax Returns

BANK STATEMENTS: Last six (6) months bank statements for all bank accounts, certificates of deposit, etc.

VEHICLES: Documents showing ownership, lease or finance, and payoff statements or other proof of payoff balance

RETIREMENT PLANS: Pension, 401K, IRA, Keogh, or other Retirement Plan - Most recent statement of current value and a plan booklet or other documentation showing the type of plan and how it operates

INSURANCE POLICIES: Copy of policy and/or declaration page in which you have an interest, including life insurance.

LAWSUITS OR CLAIMS FOR BODILY INJURY: Copy of all court papers, contract with attorney representing you, doctor or medical reports, and any police reports. If the claim was settled in the last year, provide documents.

LEGAL PROCEEDINGS: Copies of documents for all legal proceedings to which you were a part in the last two years, including wage attachments/garnishments, summonses, complaints and judgments. Documents evidencing claims or money owed to you.

DIVORCE OR SEPARATION: If within last two years, a copy of the Divorce Judgment, Property Settlement Agreement, case information statement or financial disclosures and evidence of child support or alimony payments. Proof of last six (6) months payment of child support and alimony

BILLS: Most recent credit card, store charges, utility bills, medical bills, unsecured loans, home improvement loans, auto loans, tax delinquencies, tickets, fines, criminal restitution, fraud judgments, student loans, etc.

PROPERTY NO LONGER OWNED: If you sold, gave away or transferred any property in the last eight years, provide a copy of the closing statement or other written proof

BUSINESS: Copies of last three years tax returns, financial and bank statements, accounts receivable, accounts payable, contracts and leases, books and records

GUARANTIES & NOTES: Copies of documents signed as a principal, endorser or co-maker

PRIOR BANKRUPTCIES: Discharge Order & Schedules if within last eight (8) years

We will not take any steps to protect your interests, such as filing a Petition with the U.S. Bankruptcy Court, unless and until a retainer agreement formally hiring us as your attorneys is signed by you and the fees and costs called for have been paid and credited.
